death, signed by a practicing physician or the
coroner, shall be first presented to the City Treas 146/257
urer.
Sec. 11. Burial permits shall be granted by
the City Treasurer upon the application of any
undertaker or relative or friend of the deceased.
Upon such application the Treasurer shall issue a
burial permit, specifying therein the deceased per
son’s name, color, age, sex, condition, place, cause
and date of death, name of undertaker and name of
applicant, designating the location of the grave,
which permit shall be substantially in the following
form:

City Treasurer.
Upon the burial taking place, the keeper of the
cemetery shall certify upon the permit the fact
that such burial took place, and the date thereof,
together with the particular location of the grave.
Such burial permit shall be returned by the under
taker or the person having charge of the funeral,
to and filed with the City Treasurer within three
days from the date of such burial.
Sec. 12. Any person violating any of the pro
visions of this ordinance shall be punished by a
